TLC's intake is available via a download. It is an easy process that is initiated when you click on the link below. Once you have downloaded TLC's intake, please take a few minutes to review, complete and return the application via e-mail or fax. If there are additional reports, evaluation and pertinent documents, please feel free to fax this additional information to our office. Once your intake and supporting documents have been received, a TLC senior clinician will contact you to review the next steps of initiating TLC services and to answer all of your initial questions.
